Are you are interested in becoming a client of Carolina Massage, just call and we can schedule your appointment. 
 
As of 2017, all if our patient intake forms are completed electronically.  We will send you a link to complete the form online once you have scheduled your appointment.  If you do not receive the link before your appointment, please complete the below form and either email or print it and bring it with you to your session. 
 
 
 
If you are already a member and need to change, freeze (for up to 6 months), or cancel your membership, please complete the form below and email, fax, or bring this form to the office​
 

Cancellation/Freeze policy

Please note that Carolina Massage Therapy member massages cannot be shared.  There is an option to freeze your account for up to 6 months.   If you do not unfreeze your account at the end of the 6 months, your account will be canceled due to inactivity.   Carolina Massage Therapy reserves the right freeze accounts that have been unused for 3 months.  You will be notified via email when this happens.  Carolina Massage Therapy also reserves the right to cancel a membership account at anytime if the account has not been active or hasn't been reactivated after 6 months.  If you have any remaining massage sessions after your account has been canceled, you must use those massage sessions within 90 days of your account being canceled or they will be forfeited.  Any remaining massage sessions cannot be shared.  NO REFUNDS will be issued.  NO transferring massage sessions to another person.  NO GIFT CERTIFICATES can be purchased using unused massage sessions.

Please note that there is a $25.00 fee for missing an appointment without 24 hours notice.
